Thank you for choosing Let’s Smile^™ Dental! During your first visit with us, please be at our office 15 minutes before your appointment to fill out our paperwork. If you are already a patient of one of our different specialties, we still need you to fill out the paperwork that pertains to the new specialty you will visit.

• Responsible parties, please remember to bring the following: current state ID or driver’s license and your current dental insurance card.
• Once you have filled out your paperwork you will be seen by our clinic to get your x-rays and quick check before your consultation with your doctor.
• During your consultation, the doctor will explain any work that needs to be done or any preventative care that they would like to continue to ensure you have a happy and healthy smile.
• At the end of your appointment, you will review any payment for the visit with our treatment coordinators or front desk.
• Before you leave we will review all of your upcoming appointments. We will also be sure to give you an appointment slip to take back to work or school.

We don’t want finances to hold you back from having a healthy, beautiful smile, so we have several payment options designed to accommodate any budget.

OPTION A - CARECREDIT

• No initial payment
• 0% interest if paid within term limits
• Low monthly payments
• The first payment is not due for four-six week
• No prepayment penalty with terms of up to 60 months

OPTION B - DENTAL INSURANCE

• Aetna, Assurant Health, Delta Dental, Cigna, MetLife, United Healthcare, Principal Financial Group, Guardian, Blue Cross Blue Shield, Humana
• We accept most insurances including PPO, HMO, and DHM.
• We accept Medicaid at our Alexandria, Centreville, Fredericksburg, Herndon, and Springfield locations. Contact us to determine whether you are covered for a specialty visit.
• Oral Surgery department does not accept Medicaid for adults 21 and older.
• We will be glad to file a predetermination for you to verify coverage of any necessary dental treatment.
